Functional Assessment — In-Person (Ballarat)

Comprehensive, face-to-face functional capacity assessments for autistic and neurodivergent people in Ballarat and surrounding areas

Our in-person functional assessments combine direct observation with standardised assessment tools, delivered face-to-face in Ballarat. This format allows our clinicians to observe certain skills and behaviours in real time, in ways that complement the structured interviews and standardised tools used throughout the process.

What This Assessment Covers

  • Daily Living Skills — personal care, routines, household management, nutrition, and safety

  • Community Participation — navigating community environments and engaging in meaningful activities

  • Social Functioning — communication, relationship development, and social participation

  • Executive Functioning — organisation, planning, problem-solving, and task management

  • Education and Employment — participation, accommodations, and support requirements

  • Independent Living — current and future capacity for self-directed living

  • Health Self-Management — capacity to manage one's own health affairs

How It Works

  • Clinician-led, family-informed — an initial consultation with the participant and their loved ones identifies what matters most, shaping the focus of the assessment

  • Delivered face-to-face, in Ballarat — structured interviews, direct observation, and standardised assessment tools are conducted in person

  • A report you can actually use — findings are compiled into a comprehensive written report covering functional strengths, support needs, and evidence-based recommendations — suitable for NDIS plan reviews, education inclusion plans, and DSP applications

Is This Assessment Right for You (or Your Family Member)?

An in-person assessment may be a good fit for someone who:

  • Is based in or near Ballarat

  • Is preparing for an NDIS plan review and needs updated evidence of support needs

  • Wants a clearer picture of strengths and challenges across daily life, not just a diagnosis

  • Is applying for the Disability Support Pension or another support requiring documented evidence

  • Has a support coordinator or care team who would benefit from a shared, structured understanding of needs
