Meaningful support begins with meaningful relationships.

At Unified Care Solutions, our team is united by a shared commitment to helping autistic and neurodivergent children, adolescents and young adults develop social confidence, build meaningful connections, and participate more fully in community life. We work with participants aged 4 to 35, and with the families and carers who support them.

We believe that effective support is built on trust, consistency, and genuine understanding of the individuals and families we work alongside.

Our team combines professional knowledge, practical experience, and a commitment to continuous learning to deliver supports that are both person-centred and outcome-focused.

Zac Eaton

Zac Eaton is the Founder and Director of Unified Care Solutions and a Clinical Nurse Consultant in intellectual and developmental disability support. He established UCS to bridge the gap between structured social skills training and real-world application for autistic children, adolescents and young adults.

Zac commenced tertiary studies in Social Science at Federation University before transitioning to Australian Catholic University where he completed a Bachelor of Nursing, Bachelor of Paramedicine and a Graduate Diploma in Clinical Nursing. Zac is a PEERS® Certified Provider, trained in the evidence-based social skills program developed at UCLA. He has also undertaken additional training in autism screening, clinical assessment, consultation, and functional reporting.

His clinical approach integrates developmental theory and social systems thinking, with a focus on how social communication impacts education, employment, independent living, and community participation.

Zac's clinical interests include autism, ADHD, intellectual disability, social communication, adolescent health, neuro-affirming psychoeducation, self-management, emotional and behavioural regulation, decision-making capacity, LGBTIQA+ health, and complex case management.

Zac is autistic and has ADHD. This lived experience informs his interest in practical, strengths-based, and participation-focused approaches to support.

Outside of work, Zac enjoys fishing, camping, and exploring foods from different cultures. He enjoys fishing for the experience just as much as the catch - which is fortunate, because the catch is often in short supply.

Operations Supervisor
Certificate III in Individual Support

Joseph Reyher-Cochrane

Joseph is a Disability Support Worker who holds a Certificate III in Individual Support.

Joseph has supported autistic people and people with ADHD, intellectual disability, psychosocial disability and mental health conditions. His experience includes personal care, daily living support, complex behaviours, community access and life transition planning.

Joseph describes his support style as person-centred, with a strong focus on treating every person with respect, kindness, and understanding. He believes in building genuine, trusting relationships and tailoring his support around each individual's goals, strengths, and what matters most to them. Joseph aims to encourage confidence, independence, and meaningful participation in everyday life, while being someone that people can rely on and feel comfortable with.

Joseph considers his greatest strengths to be his friendly, supportive, dependable, and understanding nature. He enjoys seeing the positive difference he can make in the lives of the people he supports and values open communication, mutual trust, and respect as the foundation of safe and effective support relationships.

Outside of work, Joseph enjoys fishing, camping, cooking, gaming, spending time with animals, and listening to music. Perhaps owing to his passion for the outdoors, one interesting fact about him is that he has been bitten by a shark!

Registered Nurse (Division 1)

Luisa Baretto

Luisa is a Registered Nurse who brings both clinical depth and a compassionate, person-centred approach to her work. She has three years of disability-specific experience in Australia, following six years as an oncology nurse prior to her move, where she transitioned into community nursing and disability support work. Luisa holds a Bachelor of Science in Nursing and has developed experience supporting autistic people, community access and social participation, as well as manual handling.

Luisa describes her support style as person-centred and compassionate, with a strong focus on understanding each individual’s needs, goals, and lived experience. She values empathy and listening without judgement and sees these as essential to building trust and delivering safe, respectful support.

What Luisa values most about working in the disability sector is the privilege of being welcomed into the lives and backgrounds of clients and their families and being trusted to support them in meaningful ways. Outside of work, she shares that she once dreamed of becoming an astronaut - a reflection of her curiosity and appreciation for learning and exploration.

Disability Support Worker
Bachelor of Nursing / Bachelor of Paramedicine (completing 2026)

Logan Weiler

Logan brings five years of experience across specialist education schools, residential aged care, and disability support. She is currently completing a Bachelor of Nursing and Bachelor of Paramedicine, with graduation expected at the end of 2026.

Logan has experience supporting autistic people and people with ADHD or intellectual disability, including complex behaviours, community access, personal care, daily living skills, youth support, and public transport training. She describes her support style as person-centred and trauma-informed, focusing on each person's strengths while helping them develop the skills and confidence to achieve their goals.

Logan says her strengths include her compassionate and caring nature, attention to detail, and ability to draw on her diverse experience to provide individualised support. Outside of work and study, she enjoys drawing, painting, and exploring different forms of art. She finds the most rewarding part of her role is supporting participants to achieve goals they have set for themselves.

Logan values respect, dignity, inclusion, autonomy, and strong professional boundaries, believing these are essential to creating safe, positive, and empowering support relationships.

Disability Support Worker
Certificate IV in Community Services

Joshua Daly

Joshua is a Disability Support Worker with over four years of experience in the disability sector. He holds a Certificate IV in Community Services.

Joshua has supported autistic people and people with ADHD, intellectual disability, psychosocial disability, mental health conditions and acquired brain injury. His experience includes personal care, daily living support, complex behaviours, SIL, and community access.

He describes his support style as person-centred and strengths-based, focused on promoting independence while respecting each individual’s choices and goals. Joshua says he builds trusting relationships, adapts to different communication needs, and remains calm and patient when supporting complex behaviours, using positive behaviour support strategies to achieve positive outcomes.

Joshua’s key strengths include his calm and patient approach, strong communication, adaptability, reliability, and ability to build meaningful relationships.

Outside of work, he enjoys camping, fishing, spending time with family and friends, and has an interest in cars and 4WDs. He says the most rewarding part of his role is seeing participants achieve meaningful goals, no matter how small.

Joshua values dignity, respect, and clear communication in all support relationships, and believes in maintaining strong professional boundaries while delivering safe, person-centred care.

Aged Care Worker
Certificate III & IV in Aged Care

Leeanne Eaton

Leeanne brings over 30 years of experience in the care sector, having first entered the industry as a Care Coordinator before transitioning into direct support roles. She holds both a Certificate III and Certificate IV in Aged Care and has extensive experience supporting people with personal care, activities of daily living, and independence-building.

Leeanne is passionate about helping people live as independently as possible throughout every stage of life, with a strong focus on maintaining capacity and wellbeing as people age. She describes her greatest strengths as her advocacy for clients, supportive nature, and ability to work independently while delivering high-quality, person-centred support.

Outside of work, Leeanne enjoys spending time with her three Golden Retrievers, walking outdoors, and reading.

Administrative Assistant
Certificate III in Aged and Community Care

Paige Eaton

Paige has three years of experience in healthcare administration and holds a Certificate III in Aged and Community Care. After completing her qualification, she discovered a passion for the challenge and complexity of administrative work, where she plays an important role in supporting high-quality service delivery behind the scenes.

Paige's strengths lie in supporting the team with the administrative processes that enable participants to receive the supports they need to progress towards their goals. She strives to be someone people feel comfortable confiding in and is committed to providing approachable, reliable support to both clients and colleagues.

Outside of work, Paige enjoys holidaying with her family, exploring new places, home styling and renovating, and spending time with her partner and young daughter. A fun fact about Paige is that she is a twin!
