Functional assessments provide a structured understanding of how a person's disability impacts everyday life. At Unified Care Solutions, our assessments go beyond documenting limitations — we identify strengths, environmental factors, and practical recommendations to help participants, families, support coordinators, and funding bodies make informed decisions.
We offer two ways to complete a functional assessment, so you can choose whichever suits your circumstances.
Choose Your Assessment Pathway
In-Person Assessment (Ballarat) — face-to-face assessment combining direct observation with standardised tools, delivered locally in Ballarat and surrounding areas.
Telehealth Assessment (Australia-wide) — the same rigorous, evidence-based process delivered via video, available to participants anywhere in Australia.
Not Sure Which Is Right for You?
Both pathways use the same standardised assessment tools and produce the same comprehensive written report — suitable for NDIS plan reviews, education inclusion plans, Disability Support Pension applications, and care planning. The right choice usually comes down to location and preference:
If you're based in or near Ballarat and have no strong preference, an in-person assessment allows for some observation methods that are easiest to conduct face-to-face
If you're outside the Ballarat region, or simply prefer the flexibility of not travelling, telehealth delivers the same depth of assessment remotely
